LONG FORM APPLICATION

You must use the long form if you are either:

(1) An attorney new to this assigned counsel panel or

(2) A currently active attorney who wishes to change either the court locations in which you will accept assignments or the level of assignments which you are approved by the program to accept.

To do so, please (1) read the instructions in the following link: Applic08 cover ltrlong online version. pdf and (2) click on the following link, print out the form on that link, and send the completed and signed form to our office at the address indicated on the application form.  We need an original signature on this form, so electronically submitted applications cannot be accepted.  Application form: Applica08 long online version.pdf

SHORT FORM APPLICATION

If you are an attorney who is currently active with the Assigned Counsel Program in Erie County, New York, and you do not wish to make any changes in either the court locations in which you will accept assignments or the level of assignments which you are approved by the program to accept, you may use the short form.
