Important Instructions for Attorneys
Long Form Application
You must use the long form if you are either:
- An attorney new to this assigned counsel panel or
- A currently active attorney who wishes to change either the court locations in which you will accept assignments or the level of assignments which you are approved by the program to accept.
To do so, please read the instructions on the following link; 2011 Application Long Form Instructions and then select this link; 2011 Application Long Form. Print and send the completed and signed application to our office at the address indicated on the form. An original signature is required. Electronically submitted applications cannot be accepted.
Short Form Application
If you are an attorney who is currently active with the Assigned Counsel Program in Erie County, New York, and you do not wish to make any changes in either the court locations in which you will accept assignments or the level of assignments which you are approved by the program to accept, you may use the short form.

Print and send the completed and signed application to our office at the address indicated on the form. An original signature is required. Electronically submitted applications cannot be accepted.
